Horse Performance Certificate Application

HELPFUL HINTS:

1. Do not submit a Horse Performance Certificate application until all score requirements have been met.
2. Please see the online USDF Member Guide for Horse Performance Certificate requirements. Questions?
3. Horse must be USDF Lifetime Registered at the time scores are earned..
4. Owner must have a USDF Participating or Business Membership at the time scores are earned.
5. You are not required to submit copies of the tests.

7. Submission of a Horse Performance Certificate application does not guarantee that the award requirements have been met.
8. USDF members with questions regarding scores available for USDF Horse Performance Certificates may contact USDF for verification or verify scores on USDFScores.com. Please note that USDF Group Members only have access current year scores on USDFScores.com.
